Adoptions, Surrenders, Shelter Volunteers & Personal Donations for the Animals, use this form.

If inquiring about a pet, please fill out the Adoption Pre-Approval Form on the Adoptable Pets pg.

If wanting to surrender a cat or dog, you MUST fill out the Cat or Dog Surrender Form.

You do not need to fill out this form unless you have not received a response from either of the above forms with in 48 hrs.

Physical donations can be dropped off at:

This form is for fundraisers, monetary donation questions, estates, billing, corporate donations/pick up of bulk goods, dog trapping, humane issue advice, etc. Do NOT use this form for adoptions, surrender of animals, or personal donations for the shelter.
