Dog Surrender Form

All * fields MUST be filled in completely. Your will not be able to SUBMIT unless complete.
ARC asks for a $50 surrender fee for each dog being surrendered.
Filling out this form does not guarantee that ARC is able to take your dog.
Dogs can only be accepted if ARC currently has room for your size dog.
Upon submission of this form, an ARC rep will contact you within 2-3 days to let you know when we expect to have room or if we have further questions.
Obtain & bring all vet records even if expired.
